Accurately input results of telephone assessment in a computer database to facilitate aggregation and analysis. Follow up with patient's physician or with the CHHA, if additional medical or home care intervention is identified. Work a flexible eight (8) hour shift five (5) days per week. JOB REQUIREMENTS: Current NYS RN license required. A diploma or degree from an NLN accredited school of nursing or nursing program, and one year of nursing experience. Home care experience preferred.
